We are a policy think tank dedicated to independent research and information exchange on health integration in Ontario.
It is not a question of how well each process works, the question is how well they all work together.

The Change Foundation's Meeting of the Minds ignites debate & yields ink, air time on health-care regionalization

The strengths and weaknesses of Canadian health-care regionalization were debated in camera and in front of it, following the launch of the Foundation's Meeting of the Minds on May 20 and 21. The invitational exchange, Lessons & Confessions from the regionalized health-care front: Where can they lead Ontario?, drew more than thirty eminent health-care leaders from across the country and featured a stellar pan-Canadian line-up. Among the speakers and participants: former Health Commissioners; regional health authority CEOs; former deputy ministers of health; CEOs from Ontario's LHINs, hospitals, and community care sectors; and senior Ontario government bureaucrats. The forum offered the latest learnings and insights on health-care regionalization for Ontario's health leaders and provoked public discussion through thought-provoking media reports. See TVO's Agenda with Steve Paikin and Toronto Star (Carol Goar) May 26 and May 28 coverage. Read presentations by Kenneth Fyke and Steven Lewis, the foundation's research advisor. See who was there and look for a report soon. The Foundation plans more exchanges and events this year.

The Change Foundation to release 1st health integration report on patient and caregiver experiences at June 12 AGM

"We need better communication. ...and communication is everything from information sharing, whether that's through computer files from caregiver, hospital, other institutions, whether rehabilitation or social services or whatever." This is just one voice that helped inform the Foundation's first health integration report -Who is the Puzzle maker? Patient/Caregiver Perspectives on Navigating Health Services in Ontario -- which will be released at its AGM next week. The report draws on focus group findings of patient and caregiver experiences navigating Ontario's health-care system as well as on survey findings and a literature review of public expectation and patient experience of integration. Come hear some of their stories and learn about our findings. Our AGM will open with a compelling video of the voices and views of patients and caregivers. A panel discussion follows, featuring Change Foundation Chair Gail Donner, President and CEO Cathy Fooks, South East LHIN CEO Paul Huras, and Central CCAC Executive Director Cathy Szabo. RSVP to our
e-vite and download our report from our website June 12.

Foundation to study fast growing area of informal care

The Change Foundation has partnered with Laurentian University researcher Raymond Pong to study how to better support the growing and essential role of informal care for the elderly within Canadian health care. 70-80% of all in-home care for elderly people with chronic impairment is provided by families or friends. The research will help identify links between support for informal caregivers and better quality of life for both recipients and caregivers. "This timely project aligns with the province's Aging at Home Strategy and the Foundation's goal to improve the quality of health care. We know that informal caregivers are already playing an important role in our system as most seniors prefer to get care at home with family and community support. This overview will help us narrow down the research questions" says Change Foundation CEO Cathy Fooks. Dr. Pong will complete his report this fall. Learn more from this presentation by Dr. Pong.

The Change Foundation leads session on physician integration at national conference

Following on the heels of the Foundation's first Meeting of the Minds exchange on health-care regionalization, Change Foundation CEO Cathy Fooks and Research Advisor Steven Lewis participated in a session on a similar topic at the Canadian College of Health Service Executive conference, Regionalization: Lessons Learned or Lessons Lost?, in Saskatoon June 2 - 3. The dynamic session, led by Lewis and moderated by Fooks, explored the barriers and opportunities to engage physicians in the regional delivery of health care. Physicians on the panel included Vern Jubber, Chief Clinical Officer, Chinook Health, and Stewart MacMillan, Head of Family Medicine, Regina Qu'Appelle Health Region. Discussion focused on strategies to improve coordinated practice and incentives to encourage more patient-centred care. Read more about key points raised in this discussion attended by over 150 health-care leaders from across Canada.
